New Patient Registration
We can only accept new patients who live in our practice area. Our catchment areas are Woodsetton and surrounding areas. Please contact reception to see if you address falls within our catchment area.
You can register by completing a registration form which can be obtained from the practice. When attending the practice for your appointment, you will need to bring with you two forms of identification such as passport, driving license or utility bill.
When you register, we will ask you to make an appointment to see the practice nurse for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure / smear tests / mammograms) are up-to-date.
